For each application, the recommended methodological approaches are discussed and the available cumulative clinical experiences of investigators from across the globe are reviewed. A conscious effort is made to highlight recent developments and innovative multidisciplinary approaches within each clinical area. Interesting issues and novel approaches are further explored through a collection of selected case reports at the end of the book.
